Now, we fill in the rest of the form. We add typewritten text blocks for address, age, etc., repositioning each text block as we go; we place a capital “X” in each appropriate checkbox (Figure 9).

So, we’re done; we’ve filled out our form. If the clinic wanted the form faxed to them, we’ll email it, instead. (I’ve yet to have anyone complain about receiving a form by email rather than fax.) In a pinch, you can just print the completed form and bring it with you to your appointment; at the very least, everyone will be impressed with how neat and tidy your form is. (This is true; I get frequent complements.)
